Medical Conditions Services

Health Condition Services are special programs for disease management, chronic illness, and health services for you and your loved ones. Support services include UnitedHealthcare (UHC) programs and Columbia on-site and virtual resources.

Whether you want to understand your treatment options or find health support, UHC Nurses are here to help. UnitedHealthcare programs are provided to Columbia employees enrolled in a UHC medical plan. Programs come at no extra cost to you as part of your plan. 

  • Get ongoing 1-on-1 support from a nurse, so you can breathe easier. When you join, you’ll partner with a nurse, who can help you manage your asthma symptoms, create an action plan and take steps to control health care costs.

  • Bariatric Resource Services is here to help guide you through your weight-loss journey as you explore options — from diet, exercise and lifestyle changes to surgery. Based on your needs, surgery is offered through CUIMC as a preferred Center of Excellence or another medical center from the Centers of Excellence network can be recommended. You’ll connect with a nurse specially trained in bariatrics for support before, during and after your treatment.

  • If cancer touches your life, this personalized program is available for you, your family and your support system. UHC’s specialized cancer nurses can help you manage your symptoms, find doctors that fit your needs, and explore treatment options based on your unique needs.

  • If you live with kidney disease, a specialized nurse is here to give you ongoing support to help you manage your condition and explore treatment options. Also take advantage of top-performing centers through our preferred network.

  • CUIMC provides seamless, multidisciplinary care for the successful treatment of adult congenital heart disease (ACHD) patients with unique issues at the Schneeweiss Adult Congenital Heart Center. CUIMC providers have expertise in issues surrounding patients with all known forms of adult congenital heart disease.   • One of the most common type of heart disease is coronary artery disease or CAD. CAD is caused by plaque buildup in the walls of the arteries that supply blood to the heart, called coronary arteries and other parts of the body.

  • Neonatal Resource Services (NRS) provides a dedicated team of experienced neonatologist & neonatal intensive care unit (NICU) nurses who can help ensure the right plan of care for the NICU infant and help facilitate a safe discharge home. Members can take part in the NRS program by calling the member phone number on your ID Card.

Columbia Resources:

  • If you need help deciding where to go for a transplant, this program may be for you. Based on your needs, a special transplant nurse can refer you to CUIMC as a preferred Center of Excellence or another medical center from the Centers of Excellence network. Plus, your nurse will work with your doctors to help coordinate your treatment before and after a transplant.
