Implementation of Online Paid Family Leave Form and Workflow

As part of our continuous commitment to simplify processes and enhance our efficiency, we have identified key changes in our workflow, including an online Qualtrics request form, to streamline Paid Family Leave (PFL) requests.

What does this mean for you?

Effective June 26, 2024, when an employee requests a Paid Family Leave, please complete the Qualtrics request form. Leave Management will receive the submission and use that information to process the leave.

Why are we doing this?

To increase consistency and efficiency of the PFL process, Leave Management is centralizing the PFL process to improve both the employee and the Department experience.

What are the steps completed by Leave Management after you submit the form?

  • Complete the Family and Medical Leave Act form, based on information from you and send it to the employee, with a copy to you.
  • Provide the employee with contact information for New York Life.
  • Answer any questions that an employee has regarding PFL.
  • Confirm use of parental care leave/vacation/personal time with the employee.
  • Leave Management will provide you with PAF codes.

What are the steps to be completed by the Department?

  • After notifying Leave Management of a Paid Family Leave, the Department will provide wage information to Leave Management as requested.
  • The Department will complete a PAF, based on codes provided by Leave Management, when notified either of a PFL approval or initially for another leave depending on the timing of the PFL request.
  • In cases where an employee is nonresponsive to Leave Management, the Department will be notified and will be asked to assist.

We aim to continue improving communication channels to facilitate the sharing of information for leavebenefits and documentation requirements. To support this new workflow, Leave Management representatives will communicate directly with employees upon receiving the request to gather documentation and process the leave request. We will also continue to incorporate additional changes including better ways to collect wage information and PAF completion.

We invite you to attend the scheduled Leave Management trainings on the following dates:

  • Tuesday, June 18: 11:30 a.m. – 12:30 p.m. | Register
  • Tuesday, June 25: 2 – 3 p.m. | Register 

During those trainings, we will offer a refresher on requirements, benefits, and further communicate the amended workflow. We will also allocate time for questions and comments. Your partnership is greatly appreciated.
